Going to the states soon, what are the requirements/permissions/conditions under which one can bring medicinal pot back into Aus if any? (i,e doctors prescription from Aus or US).

If you have a prescription from an Australian doctor for cannabis then yes you can legally bring back 3 months supply with you to Australia. Getting the prescription is next to impossible though.
